Your medical privacy rights
South County Fire respects the privacy of your protected health information. We seek to ensure that the confidentiality of your information is maintained in a responsible and professional manner.
Under the federal Health Insurance Portability and Accountability Act (HIPAA), you have the right to:
- Request a copy of your medical record in paper or electronic form. Reasonable charges may apply
- Get a copy of our full privacy policy (PDF)
- Correct your paper or electronic medical record
- Ask us to limit the information we share
- Get a list of those with whom we’ve shared your information
- Choose someone to act for you
- File a complaint if you believe your privacy rights have been violated
We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available upon request, in our office and on our web site.
